This registration form should be submitted for anyone who wishes to receive Confirmation. Parents, including parents of children who attend Queen of All Saints Academy, should fill out this form for their child. 

​

The parish office will review submitted forms and schedule classes as necessary.

Those who intend to be confirmed on May 19, 2024, must submit their registration form by March 1, 2024.
